Below are descriptions of and links to forms that are used by graduate students and graduate programs.  E-mail forms marked with an asterisk (*) to gradforms@miami.edu.

Application for Undergraduates to Take a Graduate Course

University of Miami undergraduate students within 30 credits of meeting the requirements for the baccalaureate degree may take and receive credit for graduate courses, while completing the requirements for the baccalaureate degree.  No more than six (6) graduate credits may be taken in one semester, and no more than a total of twelve (12) graduate credits may be taken as an undergraduate. The student should be enrolled in at least 12 undergraduate credits each semester to avoid eligibility issues for some aid programs. Admission to take graduate courses does not automatically admit the student to a graduate degree program at the University of Miami.  The graduate credits earned may NOT be used to meet undergraduate graduation requirements or be used to meet the 120 credit hour requirements at the University of Miami.  Undergraduate students who request to take graduate coursework must complete the Application for Undergraduates to Take a Gradaute Course and then bring the completed form and a completed drop/add form to the Graduate School Office, Ashe 235, for final approval.  Once the form is signed by the Graduate School representative, the student is notified that he/she may pick up the form in Ashe 235 and then he/she must bring that form and the associated drop/add form to the Office of the Registrar for processing.

Application for Admission to Candidacy*

A graduate student who has been admitted to candidacy for the Ph.D./Ed.D./D.M.A. degree must complete the Application for Admission to Candidacy form.  If there are any changes to the doctoral committee after this form is submitted and processed the first time, a new Application for Admission to Candidacy form must be submitted. 

Petition for Transfer of Credit*

A graduate student wishing to transfer graduate credits taken at another institution must complete the Petition for Transfer of credit form.

Petition for Leave of Absence*

Leave may be obtained by petition of the Program Director followed by the approval of the Dean of the Graduate School. Leave of Absence officially stops the time to completion clock.  A graduate student wishing to take a leave of absence from a graduate program, and return at a later date, must complete the Petition for Leave of Absence form.  

Application for Readmission to the Graduate School*

Graduate students who have not been continuously enrolled must request readmission. The student must contact the appropriate program office well in advance of registration.  If additional college work has been completed elsewhere since the last enrollment at the University of Miami, an official transcript of this will be required.  Recency of credit hour rules will apply. A graduate student wishing to be readmitted to a graduate program must complete the Application for Readmission to the Graduate School form. 

Application for Time to Degree Extension*

Time to completion starts when a student begins any program in the Graduate School, whether it be a master’s or doctoral program. All work must be completed within six years of the time of admission to graduate work, for those studying for the various master’s degrees; and within eight years for those studying for doctoral degrees.  Students requesting additional time to complete the graduate degree must complete the Application for Time to Degree Extension form.  

Certificate of Defense Approval for Doctoral Dissertation

Graduate Students who have successfully defended the doctoral dissertation must complete the Certificate of Defense Approval for Doctoral Dissertation form.  This form may not be submitted electronically.  The original form, with all signatures, must be submitted to the Graduate School's Dissertation Editor in Ashe 235. 

Certificate of Defense Approval for Master’s Thesis

Graduate Students who have successfully defended the master's thesis must complete the Certificate of Defense Approval for Doctoral Dissertation form.  This form may not be submitted electronically.  The original form, with all signatures, must be submitted to the Graduate School's Dissertation Editor in Ashe 235. 

Defense Notice Form

When you and your committee have decided on a firm defense date, time, and location, download and complete the Defense Notice Form, have your main advisor sign the form, then drop it off at the Graduate School or e-mail it to grad.defense@miami.edu

ETD Availability Agreement Form

Click here for the ETD Availability Agreement form.

This form informs the Graduate School of the availability option you want for your Master's thesis, dissertation, or doctoral essay in the UM Scholarly Repository. Download and complete the form then discuss with your main advisor what availability option is appropriate for your document in the Repository. You and your main advisor should sign the form. This form may not be submitted electronically.  The original form, with all signatures, must be submitted to the Graduate School's Dissertation Editor in Ashe 235.

Graduate Course Request Forms*

Graduate Programs that need to modify a current course or are requesting a new course must complete the Office of the Registar's forms and submit them to gradforms@miami.edu for review and signature.  The Graduate School will then send the signed copy of the form and associated documents (e.g. course descriptions, syllabi) to the Office of the Registrar.
